Download  background authorization form - above.  It is a word document so you need microsoft word. 
- Any individual who requests a background check will need to have the form completed by the evacuee.  I will not need to see the form; however, it must be signed.  What we have been doing with request from katrinahousing.org is having the request emailed to me, and I am conducting the background check and emailing the results in a reply email.  I will continue to do this for your people (KatrinaNetworkMass Network) as well.

For School-Aged Children Displaced by Hurricane Katrina

Children under 18 that have relocated to Massachusetts are eligible to receive free meals once enrolled in schools.

These children are considered homeless students, and the federal McKinney-Vento Act requires districts to enroll these students in their schools.

As homeless students, these children qualify for free meals.
 

For Families Displaced by Hurricane Katrina in Need of Food Stamps . . .

Applicants should go to the local Department of Transitional Assistance office to apply. They should identify themselves as being displaced by Hurricane Katrina. Applications for Food Stamps will be expedited, meaning that they will be processed immediately and have their benefits within seven days. They will receive an Electronic Benefits Transfer (EBT) card that same day. Once the application is approved, benefits will be transferred into the account. They will be certified to receive food stamps for 3 months, after which time they will be required to update the DTA on their situation.

The Massachusetts Department of Transitional Assistance (DTA) has relaxed its procedures for victims of Hurricane Katrina as follows:

  • Household Composition Rules have been relaxed — the hurricane victims will be considered a separate household. Income or asset information of the host family will not be counted.

  • Identification — If the head of the household does not have identification, the DTA will try to obtain collateral proof, such as someone who can attest to their identity. If that is not possible, they will accept a self-declaration or sworn statement regarding their identity.

  • Income — Unless the family is receiving a federal benefit such as Social Security to which they will have access, their income will be counted as zero.

  • Assets — Assets will not be counted.

  • Verifications — Verifications will not be required.

The DTA is working with other state and federal officials to address the special needs of hurricane victims housed at Otis Air Force base.
 

For Families Already Receiving Food Stamps . . .

If you are a family displaced by Hurricane Katrina and are already enrolled in the Food Stamp Program, you should know the following:

  • Your Electronic Benefits Transfer (EBT) card can be used in any state.

  • People currently receiving food stamps, who do not have their EBT card with them, can contact any Massachusetts branch of the Department of Transitional Assistance, or the following:

    • Evacuees from Louisiana who have lost their card and do not know their EBT card number should call the Louisiana State office at 1-866-334-8304, or the JP Morgan toll-free number for Louisiana at 1-888-997-1117.

    • Evacuees from Mississippi who have lost their card and do not know their EBT card number should call 1-866-449-9488.

    • Evacuees from Alabama who have lost their card and do not know the card number should call the Alabama State Office at 1-800-997-8888 or the ACS toll-free number for Alabama at 1-866-512-5087.

To the Boston University Community:

The tragic and terrible hurricane and the subsequent flooding along the United States Gulf Coast have saddened the entire Boston University community. We are in the process of contacting students and alumni who may have been directly affected by this disaster. To date, we have not been notified that any member of our community has fallen victim to this tragedy. We are also eager to have all of our students back at the University and to provide whatever medical, counseling, spiritual and other student support services to them as may be necessary. In addition, students, faculty and staff who have particular concerns, questions or other issues related to the tragedy are encouraged to contact either my office (617) 353-2200 or the Offices of the Dean of Students (617) 353-4126 or the Provost (617) 353-2230. The support network for students includes the following:

  • The Mental Health and Counseling Clinic at Student Health Services, 881 Commonwealth Avenue, (617) 353-3569, for professional help;
  • Marsh Chapel, 735 Commonwealth Avenue, (617) 353-3050, for students who wish to speak with a pastoral counselor, gather in fellowship or spend time in private prayer; and
  • Resident assistants, who are available each business day and on-call in each residence hall and residence area beginning at 5 p.m. and continuously on the weekend to assist students and to refer them to the on-campus support services. You may reach the Office of Residence Life at (617) 353-4380.

In additional to the enormous personal losses caused by Katrina, institutions of higher education all across the path of the storm have had their classes disrupted, including our sister institution, Tulane University. We believe it is critically important to do what we can to help, and, as a result, have decided to open our doors to Tulane students for the fall semester. Full-time undergraduate students at Tulane University are eligible to enroll tuition-free at Boston University for the fall semester. Graduate and professional students may also contact BU schools and will be considered on a case-by-case basis. The law and medical schools at BU have already arranged for several visiting students from Tulane this fall.

Tulane students wishing to temporarily attend Boston University should complete the ''Questionnaire for Tulane Students'' at this website.

I am confident I speak for all of Boston University in expressing our deepest sympathies to the victims and families affected by this disaster. We all hope that through aid from local, federal and philanthropic support the region will rapidly recover from this natural disaster.
